YOU WILL NEED THE FOLLOWING DOCUMENTS:

  1. Snap Code (via email or mail)

  2. Court Order (Custody or otherwise)

  3. Student's Shot Records*

  4. Health Record Update*

  5. Meal Status (Free/Reduced Meal Application)*

  6. Kindergarten and Athletic Students need physical/ health screening documentation

  7. Agriculture Survey*

  8. Handbook Verification

  9. Transportation Information

  10. MHS Parking Permit

  11. Housing Information/McKinney-Vento/Housing Information Form *


    If you are currently staying in someone else's home temporarily or are in transitional housing, please contact the registrar at the school for which you are enrolling for possible additional enrollment assistance. 
    If you have specific questions about the
